Health data rarely fails because the data is missing. It fails because a DHIS2 instance, an Android app, a lab system and four spreadsheets cannot agree on who a patient is or when a dose was actually given. That layer is the work.

Name-based child registration, schedule-driven dose tracking, catch-up logic and transfers between districts. A record that follows the child rather than a tally that does not.
Conflict-safe sync, QR identifiers, GPS and battery behaviour tested on the handsets field staff actually carry — because the last mile decides whether any of it works.
The national schedule encoded as rules, so the app tells the vaccinator what is due today. Due and overdue lists per union council, with caregiver reminders in local languages.
FHIR profiles and Implementation Guides, gateways that translate between DHIS2 and the outside world, HL7 v2 feeds from hospital and lab systems, and an identity strategy underneath.
